VALIYNT HEALTH GROUP
A Better Hand-Off Starts Before the Door Closes.
Valiynt is building a direct referral pathway for professionals who need a clear answer, an accountable contact, and truthful information about scope, geography, staffing, and timing.
WHO MAY REFER
One Path for the People Already Coordinating the Case
Hospitals, rehabilitation teams, case managers, social workers, physicians’ offices, veterans organizations, senior communities, community organizations, payers, employers, attorneys, and advocates may submit a preliminary referral.
PRELIMINARY INFORMATION
Enough to Evaluate, Without Collecting PHI
Use the initial form for contact information, city or ZIP, general support need, expected timeline, funding source if known, and safe callback details. Do not upload records, diagnoses, medication lists, insurance cards, Social Security numbers, or clinical notes through an ordinary form.
THE RESPONSE
A Clear Next Step
Valiynt will determine whether the request fits the agency’s active scope and service area, whether staffing appears feasible, and what secure follow-up is required. A referral is not accepted solely because a form was submitted.
PARTNERSHIPS
Build the Lane Before the Urgent Case
Referral partners can discuss service boundaries, response expectations, community education, discharge planning, and future payer or network readiness.
