Texas CLASS & DBMD Provider Choice
Getting off a Texas Medicaid waiver interest list can create a new problem: suddenly the family has forms, agencies, deadlines, and service-delivery choices to understand. This guide explains the provider-choice stage using current Texas HHSC materials.

CLASS
First choose a Case Management Agency
Under current HHSC CLASS guidance, the first formal provider-choice step after the vacancy offer is selecting a CMA. HHSC then notifies the selected CMA, and the CMA begins pre-enrollment work such as assessment, eligibility education, Medicaid application help when needed, and person-centered planning.
CLASS participants also use a Direct Services Agency for program services delivered through the agency model. Selected services may instead be self-directed through CDS when allowed.
DBMD
Choose a contracted DBMD provider
Texas DBMD waiver materials state that once an offer to apply is made, the individual chooses a contracted DBMD provider and notifies HHSC. HHSC then notifies the chosen provider, and the provider case manager begins the eligibility process with the individual.

Read the offer carefully
Use the date and instructions on the actual HHSC notice. The written offer controls the family’s immediate next steps.
Make the required agency choice
For current CLASS enrollment, select the CMA within the stated 30-calendar-day window. For DBMD, choose a contracted DBMD provider as instructed by HHSC.
Complete pre-enrollment and eligibility work
The assigned agency helps move required assessments, forms, Medicaid eligibility work when applicable, and person-centered planning forward.
Understand service-delivery choices
Some CLASS services may be delivered through an agency model or Consumer Directed Services. Ask which services can be self-directed and what employer responsibilities come with CDS.
Keep a written provider-choice record
Save names, dates, phone numbers, notices, signed choice forms, and the date each agency was selected.
Questions to ask a CLASS CMA or DSA
Do not choose based on a logo alone
- Which counties or catchment areas do you serve?
- How do you explain person-centered planning?
- How quickly do you return calls during enrollment?
- Which services can be delivered through CDS?
- How do transfers work if the family later wants another provider?
Questions to ask a DBMD provider
Understand the team and the process
- Who will be the case manager during enrollment?
- What documents should the family prepare?
- How are person-specific training needs handled?
- Which services are available through the agency model or CDS?
- Who should the family contact when enrollment stalls?

CLASS and DBMD provider-choice FAQ
Does a CLASS offer mean services begin immediately?
No. The offer begins an enrollment and eligibility process. Agency choice, assessments, planning, Medicaid eligibility work when needed, and required approvals still have to be completed.
Can a CLASS participant use CDS?
Selected CLASS services may be self-directed through CDS. The case manager is required to explain available service-delivery choices.
Can a person change CLASS providers later?
Texas guidance provides freedom of choice, and CLASS participants may request transfer to another DSA. Coordinate the transfer through the appropriate case-management contacts.
Is Valiynt on the CLASS or DBMD choice list?
No claim of that kind is being made. Valiynt is pre-license on the HCSSA Personal Assistance Services pathway and is not presenting itself as a CLASS or DBMD waiver provider.
